WHY AAHCM EXISTS
Care Is Moving Into the Home
For every person living in a nursing home, three more people just as medically complex are living at home. As the population ages, more care will happen at the kitchen table than in the exam room — making home-based primary care, Hospital at Home, and home-centered palliative care essential parts of how medicine gets delivered.
A Field the System Wasn't Built to Support
Few clinicians have been trained to provide the kind of care it requires. AAHCM was founded in 1987 (as the American Academy of Home Care Physicians) to answer that need and renamed in 2014 to reflect a membership that has grown well beyond physicians alone.
A Growing Gap Between Need and Care
Roughly two million older Americans are completely or mostly homebound, and another 5.5 million depend on others just to leave the house. Yet only about one in ten currently receives primary care at home. AAHCM exists to close that gap.

Other organizations touch pieces of home-based care. AAHCM is different: it's the only national professional society whose entire mission, membership, and purpose are built around home care medicine. AAHCM delivers on the promise of interdisciplinary, high-value health care in the home for all people in need by promoting the art, science, and practice of home care medicine — and that mission is the whole organization, not one program within it.
Built for the Whole Care Team
Home care medicine is inherently interdisciplinary — physicians, nurse practitioners, physician assistants, social workers, allied health professionals, and practice leadership all working together around the patient. AAHCM's membership reflects that reality, and its training and advocacy are built to serve everyone, not just the clinicians who lead it. Our vision: interdisciplinary health care, including home based primary care and Hospital at Home, taking place in the home, available to all in need.

As home care medicine has grown in scope and visibility, it has also outgrown old assumptions about what house calls can be. AAHCM has spent decades building the infrastructure the field needed to be taken seriously — and continues building what it needs to scale responsibly.

From Isolated to In Community
Home-based clinicians often work outside the walls — and outside the built-in community — of a traditional practice. Many AAHCM members describe what it felt like before they found the Academy: connected to medicine broadly, but isolated within their own field. AAHCM exists to give like-minded clinicians and leadership a professional home where their work is understood, valued, and shared.

Through Special Interest Groups, the Annual Meeting, and an online forum, members connect with peers who share their specific patient populations, practice models, and challenges. Through the Journal of Advanced Home Medicine and regular policy updates, they stay current on a field that is evolving quickly — and connected to a community that takes pride in being at the forefront of where health care is going.
