Quality Improvement in Home-Based Primary Care
AAHCM is proud to highlight a national effort helping HBPC practices deliver better, more consistent care to the homebound patients who need it most.
Quality is at the Heart of Home-Based Care
Nearly 7.5 million older Americans are homebound or semi-homebound — patients with complex chronic conditions and few options beyond the care that comes to them. As our field moves toward value-based models, demonstrating the quality of that care and the ability of HBPC practices to deliver that care has never been more important.
Without robust quality improvement infrastructure, home-based practices can struggle to benchmark their work, engage in performance and process improvement and make the case for the exceptional, cost-effective care they provide every day.
Over the last several years, The National Home-Based Primary Care Learning Network was built to close that gap. The Learning Network is a national quality improvement collaborative for HBPC practices. Its goal is to create a community of continuous learning that raises the standard of care for homebound patients across the country.
Over the next 3 years, with grant support from The John A. Hartford Foundation, AAHCM and the Learning Network will collaborate to transition its quality improvement functions to the AAHCM in support of the ongoing development of a HBPC practice recognition program. As part of this plan, the AAHCM plans to establish a new Quality Committee to engage in this work.
