VBC 2026

Speakers: Hank Ross, MBA; Kristofer Smith, MD, MPP
Walk through how to evaluate an ACO. Hear real-world examples focused on scaling and operationalizing value-based care, incorporating both administrative and clinical end-user perspectives. We’ll explore how organizations are leveraging a digital-first, data-first approach to align care teams, drive performance, and build infrastructure that scales with the needs of complex populations.

Onboarding: A Deep Dive into Onboarding Essentials for Providers and Staff

Speaker: Shawna Ramey CPC, CPMA; Rebecca Doleman, FNP, ACHPN

Effective onboarding significantly increases the longevity of new hires, which is key to the success of all types of practices, especially those serving very sick patients in their homes. This pivotal piece of practice management is often rushed through and not given the attention it deserves. In this session we will walk through the steps needed to ensure successful onboarding for everyone from front office staff to providers and will provide actionable intel for enhancing the process, meeting the needs of your team, and understanding where they are clinically.

Safety in Home-Based Primary Care: Protecting Patients, Clinicians, and Teams

Speakers: Rachel Miller, MD; Katherine Wang, MD
Providing care in the home presents unique safety challenges that extend beyond those encountered in traditional clinical settings. This interactive workshop explores physical safety, emotional safety—including bias and moral injury—and patient safety through real-world cases, reflection, and small-group discussion, providing participants with practical tools to support themselves, their teams, trainees, and the patients they serve.

Opportunity Knocks: Welcoming the Future of Hospital at Home
This pre-conference will gather leaders and clinicians from Hospital at Home programs for the nation's premier digital Hospital at Home (HaH) meeting.

Programming includes an up-to-the-minute policy update, a forward-thinking keynote from healthcare innovator Joseph Smith, MD, PhD, two concurrent tracks for newer and more experienced programs, networking, presentations of cutting-edge research, special sessions, awards showcasing the best of the HaH community, and more.

*Please note that this pre-conference requires separate registration.
